Hi there everyone, the other day I was at ward office and I remembered how difficult it used to be to fill out a form when I first got here so here is what some of the things written of forms mean.

  • Sex   M/ F

M is for male and F is for female

  • D.O.B

This means date of birth. Remember that in the U.K we often give the month before the day so if your birthday is June 5th 1998 it would be 06/05/98

  • Name
  • Given/ First (name/ name)

This means your first name and middle name if you have one

  • Family/ Surname/ Last Name

All of these mean your family name

  • Mr
  • Mrs
  • Miss
  • Master

Mr. is for men Mrs is for married women, miss is for unmarried women and girls and master is for boys

  • Passport
  • current visa ( spousal, work permit, working holiday, student)
  • Insurance Number

You need this if you are going to work in the U.K

  • Address

Remember to start with flat number, building name, area, larger area, city/ county, and country

  • country of origin

みなさんこんにちは。前に初めて区役所に行った時、書類を記入することがなんて難しいんだと思ったことを覚えています。ですので、書類に書いてある単語の意味を説明しようと思います。

Sex   M/ F/性別
Mは男性、Fは女性です。

D.O.B
誕生日のことです。イギリスでは、誕生日の日にちの前に月を書くことを覚えていてください。ですので、もしあなたの誕生日が、1998年の6月5日なら、06/05/98と書きます。

Name/名前

Given/ First (name/ name)
これは、名前と、もしあるのならミドルネームを意味しています。

Family/ Surname/ Last Name
３つとも苗字のことを意味しています。

  • Mr
  • Mrs
  • Miss
  • Master

Mrは男性、Mrsは結婚している女性、Missは結婚していない女性と女の子、そしてMasterは男の子のことです。

パスポート
ビザ（婚姻ビザ、就労ビザ、ワーキングホリデービザ、学生ビザ）
インシュランスナンバー
もし、イギリスで働く場合はこれらのものが必要になります。

Address/住所
アパートの号数、名前、区域、地名、町の名前、国名の順番で書きます。

country of origin
出身地
